Magnesium oxide is everywhere, and it’s cheap. On the label it looks generous too, with more elemental magnesium per gram than any other common form. The catch is that your body absorbs relatively little of it. The US National Institutes of Health lists oxide among the lower-bioavailability forms, and a comparison of commercial preparations found it absorbed less well than citrate.
That gap between what the label promises and what you absorb is the whole story of this form.
Why so little gets in
What isn’t absorbed doesn’t just sit there quietly. It draws water into the bowel, which is why oxide turns up in laxatives and antacids, and why a decent dose sends people to the bathroom in a hurry. The same property that makes it a poor supplement makes it a serviceable laxative.
What it’s genuinely good for
Short-term constipation. This is the one solid use. It works, it’s cheap, and pharmacies sell it for exactly this.
Indigestion. Magnesium oxide is a common antacid ingredient, which is a legitimate role rather than a supplement claim.
Cost, if absorption isn’t the priority. Some people take a larger dose of a cheap oxide product and accept that a smaller proportion gets in. That’s a defensible trade if money is tight, though you’ll be closer to the dose where the laxative effect kicks in.
The cramps trial worth knowing about
Oxide’s reputation is poor enough that a decent result is worth flagging. A 2021 randomised, double-blind, placebo-controlled trial across multiple centres gave 184 adults aged 45 and over a 226mg magnesium oxide monohydrate supplement daily for 60 days. The magnesium group had significantly fewer night-time leg cramps than placebo, shorter cramps, and better sleep quality.
Two caveats keep this in proportion. It used a monohydrate formulation selected for better cellular absorption, so it isn’t necessarily the bargain-bin oxide on a supermarket shelf. And it sits against a broader picture, summarised on the cramps page, where a 2020 Cochrane review found magnesium unlikely to help ordinary cramps. One good trial doesn’t overturn a systematic review, but it does mean “oxide never does anything” is too strong.
When to pick something else
If your goal is raising your magnesium levels, a better-absorbed form does more with less. Citrate is barely more expensive and considerably more useful. Glycinate costs more again and is the gentlest on digestion.
The other thing to watch is oxide hiding inside blends. Some cheaper products lead with a well-absorbed form on the front of the label and mix oxide in further down the ingredient list, which lifts the headline magnesium number without adding much you can use. Read the ingredients rather than the front of the bottle.
Dosing basics
If you’re using it as a laxative, follow the packet, and treat it as short-term relief rather than a standing habit.
If you’re using it as a supplement, compare the elemental magnesium per serving as usual, then adjust your expectations downward for the absorption. Loose stools arrive sooner with oxide than with gentler forms, and that’s your ceiling whether you like it or not.
The usual cautions
The same three groups, on every page, because they matter every time. If your kidneys don’t work well, magnesium supplements can be risky, because the kidneys are what clear it: talk to a doctor first. If you take regular medication, magnesium interacts with several drugs, including some antibiotics, and a pharmacist can check yours in about a minute. If you’re pregnant or breastfeeding, ask your midwife or doctor before starting anything.
Common questions
Is magnesium oxide useless? No, it’s just badly matched to the job most people buy it for. As a laxative or antacid it’s fine. As a way to raise your magnesium, it’s inefficient.
Why is it in so many supplements? It’s cheap and it’s dense in elemental magnesium, so it makes label numbers look good at low cost.
How much is absorbed? Less than the better-absorbed forms. Published comparisons put it at the bottom of the pile, though exact figures vary by study and formulation.
